Unkind or harmful words can also be said by someone who means well and is just clueless.
I once overheard my now-ex-husband "explaining" something I did to a mutual friend. I was so mad! He totally mischaracterized my motives and made me look bad. He was so hurt when I confronted him, saying he was only trying to help. The closest I could get to getting him to understand was to say that I'd rather have someone mad at me for something I'd actually done than for something he said that wasn't even true.
The friendship with the other person has never been the same. *sigh*
